Access Mental Health Support
For intake and access to child and youth mental health services in Niagara, Pathstone supports children, youth and families. For children and youth under 18, living in Niagara, in need of mental health services, connect with us directly and one of our intake social workers will support you to access the type of programming that best suits your needs and goals.
Pathstone Mental Health is the intake and access point for:
- Pathstone Mental Health Services and Supports
- Niagara Health Outpatient Child and Youth Mental Health Clinic
- Centre de Santé Services and Supports.

There are two ways to complete a self-referral to child and youth mental health services in Niagara.

Call Pathstone Directly
One of our intake social workers will support you through the intake process. Through conversation, they will learn about your child/youth’s strengths, needs, challenges, and to understand your goals and objectives for treatment.
Once this information is gathered, you will be referred to the most appropriate program/service for your needs.
After the intake, you will receive a copy of the information that was documented during your call and information about the recommended program. Included in this package will be information about the program as well as contact information of the most appropriate person should you need to connect prior to starting your program.

Complete the online self-referral form
Please provide as detailed information as possible for each of the categories, this will assist us in understanding you/your child/youth’s strengths, needs, and which programming would be a best fit.
Please note, that once submitted one of our Intake Social Workers will call you to review the information submitted to ensure its accuracy and also ensure our understanding of your goals and objectives.

Pathstone Mental Health is committed to improving services to families. Part of achieving this goal is through research. Currently, we are partnering with Western University to better understand family experiences. We invite you to participate and hope that you will support this project by completing the survey. We hope this will contribute to advancing our knowledge and improving quality of care.
It is your choice to take part in this study. Your choice does not impact the services you receive at this agency. If you agree to participate and then decide you do not want to continue, you can stop at any time.

Dr. Agnihotri’s Paediatric Health and Wellbeing Clinic.
Dr. Agnihotri is a Community Paediatrician with a focused interest in child development, behaviour, and mental health. Dr. Agnihotri has advanced training in paediatric mental health, the diagnosis and management of ADHD, autism, anxiety, depression and trauma. This practice is now offered at Pathstone Mental Health two days a week and will focus on the 6 pillars of Lifestyle Medicine: Sleep, Physical Activity, Nutrition, Social Connectedness, Stress Management, and the avoidance of Risky Behaviours and Substances. If you believe your child would benefit from a consultation with Dr. Agnihotri (and is age 12 or under), please consult with your primary care provider who can facilitate the referral.

Access to Psychiatric Consultation
Psychiatric consultations are provided through Niagara Health. This is a one-time consultation for children and youth from age 3 up to their 18th birthday, specializing in diagnostic clarification and pharmacological treatment with the understanding that the referring primary care provider is responsible for the implementation of recommendations.
The psychiatric consultation requires a primary care referral and is NOT an ongoing counseling/therapy program. On-site services are available at the St. Catharines Site and via video-conference at the Niagara Falls and Welland sites.
If you believe you/your child/youth requires a psychiatric consultation, please consult with your primary care provider who can facilitate the referral.
